Growmore MSO runs the Medicare Annual Wellness Visit program for primary-care practices. We reach your patients, complete the health-risk screening by phone, and hand your provider a prepared report — so the visit itself is short, well documented, and focused on what each patient needs.
The Annual Wellness Visit is a yearly Medicare preventive visit. It is not a routine physical exam. It is a planning visit: the provider and care team review the patient's health risks and build or update a personal prevention plan.
Medicare Part B covers one wellness visit every 12 months, once a patient has had Part B for more than a year and has not had a “Welcome to Medicare” visit or a wellness visit in the past 12 months. Under Original Medicare, when the provider accepts assignment, the patient pays nothing for the visit itself and the Part B deductible does not apply. Medicare Advantage plans cover the visit too, under each plan's own rules. Other services provided the same day may carry their usual cost.
Many eligible patients never have one, usually because nobody had the time to call them and prepare. That is the part we do.
Health plans and Medicare score practices on quality: screenings completed, risks addressed, medications reviewed, and how patients experience their care. Many of those measures come down to questions a wellness visit already asks. A well-run wellness visit is one of the most dependable ways to find open care gaps and to document that they were addressed.
The provider stays in charge. Growmore prepares, documents and follows up. Your provider decides what to order, completes the visit with the patient, and signs. Your practice bills for the visit.
The portal identifies each patient by a study code. Full names and dates of birth are not stored here.
Working from your patient list, we confirm which Medicare patients are due for a wellness visit.
A Growmore coordinator reaches each patient by phone and completes the full health-risk assessment and medication review.
A provider report with findings and recommended actions, and a patient report with a five-year prevention plan.
The provider sees the patient — in person, or by video visit where Medicare allows it — reviews the report and signs. Your practice bills Medicare or the patient's plan, and we keep tracking the follow-ups.
Review and accept reports, update where each visit stands, print reports in batches, see estimated wellness-visit revenue, and message our coordinators — in one place, for the provider and the office staff.
Please do not send patient information by e-mail or text. Patients with a question about their wellness visit should call their doctor's office.
